Model D-1. Hospital Visitation
(a). When a health-care facility restricts a patient's visitors, the health-care facility shall allow every patient to name those individuals whom the patient wishes to allow to visit, unless:
  1. no visitors are allowed; or
  2. the facility decides that the presence of a particular visitor named by the patient would endanger the health or safety of a patient, or would endanger the primary operations of the facility.
(b). If a patient with whom visiting is restricted has not made the designation described in subsection (a), and if the patient has not stated that she or he wishes no visitors, the facility must allow the patient's domestic partner, the children of the patient's domestic partner, and the domestic partner of the patient's parent or child to visit, unless:
  1. no visitors are allowed; or
  2. the facility decides that the presence of a particular visitor named by the patient would endanger the health or safety of a patient, or would endanger the primary operations of the facility.
Model D-2. General Nondiscrimination
The [city or name of other institution] will treat people who are related as domestic partners in the same way it treats people who are related as spouses.

Model D-3. Residents Only Limit
[Add this section to your domestic partnership registration law if only those who live or work in the city can file official declarations.]

Section [fill in number]. The [name of official who will file declara-tions] will only file a Declaration of Domestic Partnership if:
  1. the partners have a residence in [name of city or county]; or
  2. at least one of the partners works in [name of city or county].
[Then add the following to the end of the declaration form:]

To be able to file this form with the [name of official], one of you must work in [name of city or county] OR both of you live together in [name of city or county] explanation below).

You don't have to check either space if you finish setting up jour domestic partner-ship by getting this declaration notarized.
